Our care – supporting people to be well, independent and at home

Reablement care is a short-term support service designed to help you regain independence after a hospital stay, illness, or fall. The goal is to help you relearn daily activities such as cooking, washing, dressing, and moving around your home, so you can return to living independently.

What does reablement involve?

  • A team of NHS and social care professionals (including nurses, occupational therapists, physiotherapists, speech and language therapists, social workers, doctors, and carers) will work with you to assess your needs and set a personalised plan.
  • Support is usually provided for 1 to 2 weeks, but you may be eligible for up to 6 weeks of free care, depending on your local area and your progress.
  • Care may include help with getting dressed, preparing meals, moving safely, and practising daily tasks until you can do them independently.

How to access reablement care:

  • If you are in hospital, staff will assess your needs and arrange care before you leave. Speak to your discharge co-ordinator to ensure this happens.
  • If you have been discharged and care has not been arranged, contact your local social services for support.
  • If you have had a fall or short illness and want to avoid hospital, speak to your GP surgery or social services. They may arrange a home visit to discuss your needs.
  • If you find everyday tasks difficult, you can apply for a needs assessment at GOV.UK to identify the right support or equipment.

What happens when aftercare finishes?

  • Your team will work with you and your family or carers to plan next steps, which may include ongoing home help, information about other support, and advice on what to do if you need help again.

Cost: Most reablement care is free for up to 6 weeks if available in your area. You may need to pay for some types of ongoing care after this period.
