South East Derbyshire Provider Collaborative

We are a collaborative of services aimed at adult mental wellbeing across South Derbyshire and Erewash but not restricted solely to these areas.

The Southeast Derbyshire Provider Collaborative is a group of grassroots providers working together and supporting each other to deliver high quality experiences.

The Provider Collaborative partners are Blue Tonic, Elephant Rooms, Honey Hope Farm (Formerly Helping Hooves) Long Eaton Community Garden and Whispering Trees .

The approach is holistic, person centred and relational rather than clinical.  It helps people get more control over their healthcare, to manage their needs in a way that suits them. It can especially help people who:

  • have one or more long-term condition
  • need support with their mental health
  • are lonely or isolated
  • have complex social needs which affect their wellbeing

Currently available funded capacity is presented below. Free person-centred evaluations are available for all referrals to help identify suitable activity options.

Referrals: For new participants to the Collaborative, we feel it is important that they are properly introduced to the activities.  We ask that before they are referred a discussion is had with the relevant Collaborative Partner and an appointment made for their first visit.  In this way we can ensure that the activity is appropriate, that there is capacity available for them to attend the activity and that the participant is given a great first-time experience at our facilities. Please submit a referral via the blue button and we will be in touch.

Transport:  We have funding to assist where travel is a potential barrier to access the services provided by the Collaborative.  Please make enquiries where this may apply through the individual Collaborative Partner or the contact details below.  (Transport funding provided by Derbyshire Mind in partnership with Derbyshire County Council).
