Holgate PCN internal Social Prescribing Weight Management platform.
Holgate PCN practice staff referrals only at present.
All patients referred via this method will be coded with relevant weight management SNOWMED code as soon as the referral has been processed.
Patients will be assessed and supported by a Social Prescriber, through a variety of in-house or community support options suitable to their needs and goals; some offers are 121, some in group settings. We are not dieticians, do not have access to interventions such as supplements or injections, and cannot provide any medical/clinical guidance. Service access criteria: Inclusion Criteria: 18+, must have capacity to be referred into the service and must have 'social prescribing for weight management' need identified by GP. Must have no health/medical needs that mean they are unsafe to follow SP or community guidance. Exclusion criteria: Emergency or crisis needs, social care crisis, intoxication, unable to engage with social prescribers or pose risk to social prescribers.
