Community Connectors is for Slough residents, patients and carers whose health and wellbeing could be improved through community-led support. The team offers one-to-one telephone support and helps connect people with local social groups, activities, community services and wellbeing, care and support services.
- Talk through what support might best meet your needs
- Find local social groups, activities and community services
- Get signposted to wellbeing, care and support services
- Get help with wider social issues when they affect wellbeing, such as food poverty, employment issues or mental health
- Receive signposting support, not statutory care, safeguarding, risk assessment or case-holding
