Young Persons Social Prescriber - Peterborough

Referral Criteria:

·         The young person must be a resident of Peterborough City and/or the household pay’s their council tax to Peterborough City Council Maps-PboroUnitaryAuthority

·         The young person must be aged between 11-18 years of age. 

·         Eligibility  to receive support if the young person is experiencing low to moderate difficulties based around low mood, anxiety, unhealthy/ risky behaviours, identity issues, loneliness /social isolation, weight / physical health. 

·         Consent must be obtained from the parent / carer (for those under the age of 16) and young person to comply with GDPR regulations and to also maximise the likelihood of future engagement.

·         The Young Persons Social Prescriber will contact the family/young person to discuss the service.

Eligibility will be assessed at the first point of contact by the Young Persons Social Prescriber.

Completing this referral form does not guarantee acceptance into the service.
