Social Prescribing is a free and confidential service that looks at the things that are important to you and supports from a ‘person-centred’ perspective in a non-clinical way.
A Social Prescribing Link Worker will take the time to listen to you, understand what you would like to improve, and help you set realistic goals if required. They can support you to make positive changes that improve your day-to-day life.
Your Link Worker can work with your GP, local services, community groups, charities and other organisations to help you find the right support in your area. They can also help you understand what services are available and how to access them.
The support will be based around your individual needs. This could be as simple as putting you in touch with a service that can help, or you may receive ongoing 1-1 or group support for a number of sessions considered appropriate for your request. This number is discussed and agreed upon referral acceptance.
Who can use the service?
- Patients registered with Marlow Medical Group
- Adults aged 18 and over
- Patients who are looking for support in their everyday lives and understand this type of referral is not for clinical review or diagnosis.
How can Social Prescribing help?
Social Prescribing can help with many different areas of life, including:
- Carers: Advice on sourcing practical, emotional, and social support for unpaid carers.
- Finding the right package of care (POC): Advice and guidance on how to source an appropriate carer or PA (Personal Assistant) for you, or a loved one.
- Housing, Benefits and Money: Finding advice and support with housing, benefits, social care and financial difficulties.
- Work, Training and Education: Exploring jobs, training courses, volunteering, education and learning opportunities.
- Arts and Creative Activities: Discovering music, arts and other creative activities that can support enjoyment, confidence and self-expression.
- Social and community connections: Finding local groups, befriending services, counselling and other sources of emotional or social support.
- Volunteering: Looking for volunteering opportunities that suit your interests, abilities and personal goals.
Most importantly, Social Prescribing is centred around you — your needs, your goals and the things that matter to you. We are here to listen; offer support and help you find the right options for moving forward.
It is important to note that we are not an emergency service provider. Although we aim to review and triage our new referrals promptly, we cannot guarantee your request will be seen the same day.
If upon review, your request is considered outside of our social prescribing remit your referral will not be accepted.
