Community Connectors – Berkshire East

Mental health support for adults in Berkshire East who are experiencing significant mental health problems that are affecting their life and wellbeing. At the first appointment, a practitioner will talk through what is happening, help understand needs, and make a plan for support. This can include practical advice and guidance, help to access community resources, brief support with stress and anxiety, links to mental health services or therapy, medication support from a mental health pharmacist, and help with housing, finances, drug and alcohol services, and employment. Eligibility: Available to residents of Berkshire East, Bracknell Forest, Slough, and Windsor and Maidenhead who are aged 18 or over and experiencing significant mental health problems that are impacting life and wellbeing. To access the service, you must be registered at one of the named GP surgeries listed on the page. Referral is via your GP or Practice Nurse to the Mental Health Integrated Community Service (MHICS) team. How to access: Ask your GP or Practice Nurse to refer you to the Mental Health Integrated Community Service (MHICS) team.
