About the NHS Diabetes support provided by Oviva
Oviva has partnered with the NHS to deliver a programme for people living with Type 2 diabetes. The Oviva Diabetes Support programme is a 12 week behavioural change programme that will support and educate you to improve your blood glucose levels, better manage your weight and reduce your risk of diabetes-related complications.
On average, participants lose 4.3kg in weight and see a reduction of 12.8 mmol/mol HbA1c. 1 in 4 people on average achieve diabetes remission using the programme.
Who can join?
- People registered with a GP practice in an area where Oviva is available
- People aged 18 or over
- Confirmed diagnosis of type 2 diabetes
What happens?
On the Oviva NHS Diabetes support programme you’ll be matched with a coach who will provide expert healthcare, with tailored support through every stage of the programme. You will have the option to choose how the programme is delivered to best suit your needs.
Speak to your GP to see if you are eligible for a referral on the NHS Diabetes support provided by Oviva. Alternatively you can self refer to the programme by completing the short form at www.oviva.com/diabetes-support
