About this service:

Social prescribing is when health professionals refer patients to support in the community, in order to improve their health and wellbeing.

Link workers give people time, focusing on 'what matters to me' and taking a holistic approach to people's health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Wokingham East PCN supports patients across the following GP practices:

  • Wokingham Medical Centre
  • Burma Hills
  • New Wokingham Road
  • Woosehill


Service access criteria: Must be registered at: Wokingham Medical Centre/ Burma Hills/ New Wokingham Road/ Woosehill. Referrals from a GP only; 18+; not with a complex mental health illness
