Points to bear in mind when referring cases to Adult Social Care:

·         There are eligibility criteria to determine if the service user is eligible for Social Care support – Care Act 2014;

·         The adult needs care and support arise from or related to a physical or mental impairment or illness not caused by other circumstantial factors (such as an accident that does not result in a long-term disability)

·         Unlike Healthcare, Social Care is not free and residents are subject to a financial assessment. If client has more than £23,250 in savings, they will be assessed as self-funders and expected to fully pay for the services, anything under this threshold, they might be expected to pay a contribution;

·         Consent and information sharing; when referring cases to Social Care, it is important that the individual consent to the referral. If the individual is not able to give consent, please inform us of the reason to why and seek consent from authorised representative (LPA) if there is one.

·         Request for Residential or Nursing homes: please note that we will always look for the least restrictive option prior to considering a placement.

Equipment: not all of the equipment is provided by the Local authority. 

Examples:

·         Recliner chairs: If the need for a recliner chair is to manage a health condition, please refer to a District Nurse (skin integrity or oedema)

·         Grab rails – this can be provided by Elders Voice or ICCS.

·         Equipment for fully funded by health service users – please contact CCG - brentccg.chc@nhs.net. If the request is for major adaptations, we can assess the service user for this to be considered, even if fully funded by NHS.

·         Telecare for self-funders – we can advise but this will have to be purchased privately

·         Equipment for care homes – we can provide equipment for residential care homes but not for nursing homes. The only equipment that can be considered is bespoke equipment that cannot be purchased by Nursing homes.  

·         We do not provide mobility aids- please refer to Brent Rehab.

Useful information:

Falls: If a client is starting to experience falls, please consider referring the client to the falls clinic prior to referring to the LA. It is important to understand the reason for the falls so that support can be provided accordingly. The LA will not be able to prevent a fall and, therefore, it would be very helpful if the relevant professionals can be involved prior to coming to us.
