Our Home from Hospital Service is designed to help people readjust to being at home after being in hospital, by supporting them with day to day tasks such as shopping & helping them reconnect with their community & other support services.
Supporting people aged 55 and over, within their own homes after being discharged from hospital.
Volunteers provide practical support and companionship to promote independence and build social connections.
Engaging people with their communities and social activities.
Transportation
Shopping, Social and Medical appointments.
Active signposting to other organisations
Assisted discharge
Taking people home from hospital & helping them settle in at home.
Service access criteria:
Predominantly over 55, but exceptions can be made in cases of complete social isolation. Must have been in hospital overnight within the last 6 weeks. Must be able to get in & out of a car for Assisted Discharge service.
