Health and Wellbeing Coaches work alongside Social Prescribing Link Workers and Care Coordinators. This role is part of the NHS’s long-term plan to make personalised care the ‘norm’ across primary care. The idea is to help support Primary Care Network (PCN) staff in helping people look after and manage their own physical and mental health.

A coach’s main role is to help people to take an active role in looking after their own health and wellbeing. Particularly people with long-term conditions that can be managed through lifestyle changes. Coaches aim to help people make these positive lifestyle changes, with the aim of leading to long-term behaviour change. This can be achieved through setting goals and targets, establishing healthier habits and increasing peoples knowledge, skills and confidence

Service access criteria: Suitable if: • Patients > 18 years old • Scores < 10 on GAD / PHQ OR not assessed by GAD/PHQ but display mild/moderate depression/anxiety, low mood, feeling ‘stuck’, overwhelmed, underlying perceived barriers to progression etc. • Scored > 26 on MoCA • General forgetfulness, overcoming difficulties, exploring options to self-manage, & encourage engagement with specialist resources • Increasing activation & engagement in supported self-management. Preparing for referral to specialist support & reviewing progress, exploring resource options / networks • Physical disability – building self-management & resilience of LTC – exploring options, resources, sources of support etc. • Pain management – non-clinical coping strategies etc. • Scores < 19 on AUDIT-Seeking to address isolation, increase resources, improve routine, sleep/life balance, address barriers to progression, behaviour change etc. • Support with underlying issues preventing progress, addressing motivation /readiness to change, overcoming barriers, supporting small steps & preparing for specialist referrals (1SS, Healthier Devon etc.) to maximise effectiveness & review progress.
