
Falls Prevention Team Criteria
Inclusion Criteria
55 or over
One or more falls in the last 12 months OR becoming increasingly unsteady
Independently mobile with or without walking aids: stick, crutch, four wheeled walker (outdoors)
Able to access the community independently (this can be with difficulty)
Independent with activities of daily living/managed
Normal to mild cognitive loss (must be able to independently complete a home exercise programme)
Fear of falling
Willing to engage in motivational conversations regarding falls prevention
Exclusion Criteria
Under 55
Unable to manage activities of daily living independently e.g. requires MDT management
Use of WZF, four wheeled walker or wheelchair for indoor use, or requires assistance to walk outdoors
Unable to access the community for community provision e.g. housebound
Does not wish to engage in motivational conversations regarding falls prevention
Anyone unable to participate for medical reasons/contraindication to exercise
Care home residents (seen within CTT)
Primary neurological diagnosis or suspected under investigation/requires investigation – this must be completed prior to assessment
Vestibular diagnosis with no other multifactorial or balance problems
