MyHealth Hillingdon is an education and empowerment programme designed to help people living with long-term health conditions take control of their health and wellbeing. We offer a range of workshops facilitated by health professionals, available both online and face-to-face, to provide the latest knowledge and practical advice.
Our programme is co-produced with service users, carers, clinicians, and community partners to ensure high-quality, evidence-based care tailored to the needs of Hillingdon residents. We focus on:
- Helping people better manage their long-term conditions
- Promoting self-management through meaningful workshops
- Reducing unnecessary GP and A&E visits by empowering individuals with education
