Social Prescribing
Social Prescribers are based within GP practices and work closely with GPs and other healthcare professionals to support patients with non-medical needs. We help people identify "what matters to them" and work together to develop a personalised plan to improve their health, wellbeing and quality of life.
This may include signposting or connecting individuals to local community groups, voluntary organisations and statutory services. Support may include help with social isolation, financial and benefits advice, housing concerns, physical activity, volunteering opportunities, wellbeing groups and much more.
This service is available only to patients registered with:
Ashlea Medical Practice (Linden & Gilbert House)
Eastwick Park Medical Practice
Fairfield Medical Centre
The Molebridge Practice
Please note: Referrals are no longer accepted via the Joy Marketplace. If you are a patient of one of the practices listed above and would like support from the Social Prescribing service, please contact your GP practice directly to discuss a referral. Alternatively, you can contact the Social Prescribing team by emailing dohel.dhclhsp@nhs.net or calling 01306 771125 (Option 2).

Patients cannot use the service if they are:
Under 18 years old
Currently receiving end of life care
Living in a residential or nursing care home
