The Community Neurological Service (CNS) in Newham is a multidisciplinary team offering specialist rehabilitation and management for adults with neurological conditions, including stroke. Our team includes physiotherapists, occupational therapists, speech and language therapists, and psychologists, working together to help you regain independence and improve your quality of life.
Therapy is provided in your own home or at community locations, including our neuro rehabilitation gym at East Ham Care Centre. We aim to:
- Deliver time-limited, specialist neurorehabilitation tailored to your personal goals.
- Empower you and your carers to manage your condition confidently.
- Support smooth and timely discharge from hospital, ensuring continuity of care.
- Provide ongoing specialist advice and support to patients and professionals in the community.
- Offer education and information to help you make informed decisions about your care.
Early Supported Discharge (ESD) Service: For stroke patients, we offer a modified ESD pathway, providing intensive rehabilitation at home for up to 2 weeks, with therapy comparable to inpatient care. Patients are seen within 24 hours of discharge (excluding weekends) and receive at least 3 hours and 45 minutes of therapy per week. A further 8-week community stroke pathway is available for less intensive support.
Our neuro rehabilitation gym at East Ham Care Centre allows you to access specialist equipment and progress your rehabilitation in a supportive environment. Outpatient clinics are also available to improve access for service users.
Who can access the service?
- Adults (16+) with a confirmed neurological diagnosis or stroke.
- Residents of the London Borough of Newham, registered with a Newham GP.
- Medically stable and able to benefit from specialist neuro input.
- Willing to participate in a rehabilitation programme.
How to refer: Referrals are accepted from health and social care professionals. Self-referrals are possible once you are known to the service. For ESD, referrals are accepted from acute hospitals only (usually stroke units).
