**Making Changes Programme - Blackpool** The Making Changes programme is designed for families residing in Blackpool, aiming to promote healthier choices and a more active lifestyle. This initiative is ideal for families with children aged 4 to 11 years, particularly those identified by healthcare professionals or school nurses as potentially benefiting from education on healthy eating and active living. **Key Features:** - **Duration**: A 12-week programme featuring: - Education on healthy eating and lifestyle changes. - Engaging themed activities. - **Family Health Checks**: Participants receive health assessments at the start (Week 1) and end (Week 12) of the programme. - **Active Blackpool Vouchers**: Families are provided with vouchers for activities, such as free family swimming. - **Ongoing Support**: Participants receive continuous guidance throughout the programme. - **Leisure Membership**: Upon completion, families are awarded a free 3-month leisure membership. - **Graduation Event**: Families celebrate their achievements at a graduation event. - **Membership Rate**: After completing the programme, families can join Active Blackpool at a reduced membership rate, effective 3 months later. **Eligibility Criteria**: The programme is open to families of children who: - Have received a referral from the school nurse regarding the National Child Measurement Programme (NCMP). - Consume little or no fruit and vegetables. - Regularly consume high-fat or sugar-rich foods. - Have fussy eaters. - Have a Body Mass Index (BMI) at or above the 91st centile (priority given). To assess your child's BMI centile, please use the NHS BMI calculator. The child & family management service requires personal data to ensure we provide you the best possible service.   

The personal information on this form will be retained to demonstrate that consent was sought for the named person above and their family to take part in this programme.  We will use the information provided to contact you in an emergency and ensure that staff delivering the sessions are aware of any medical or special support needs. 

The lawful basis for the Council to collect and use your personal information is because you have given us clear consent. 

We will collect the following information: your name, date of birth, medical information, contact details, medical history, height/weight/BMI and details of any medication. 

Your data will be stored securely and reported anonymously to Public Health.  We do not transfer your information to a third country (that is a country outside of the EEA).
