Care Navigation

Care Navigation from Age UK Redbridge, Barking & Havering is a free service designed to support older people living with multiple health conditions in Redbridge, Barking & Dagenham, and Havering.

Our Care Navigators work closely with you to understand your circumstances, hobbies, wishes, and goals. Together, we create a personalised plan to help you access the support and services you need.

  • Help accessing local services and social groups
  • Benefits advice and information
  • Signposting for legal advice
  • Support with transport schemes
  • Connecting you to activities and interests

We aim to help you enjoy a good quality of life, build confidence, and maintain independence. Whether you want to get out and about, take up new activities, or simply enjoy hobbies at home, our service is here for you. For those less able to leave home, our volunteers offer companionship and social interaction, helping to open up new friendships.

Who can use this service? Anyone living in Redbridge, Barking & Dagenham, or Havering who is experiencing social isolation or managing long-term health conditions. Referrals can be made by GPs, professionals, family, or by yourself.

Cost: This service is free to all eligible clients.

How to access: Call us on 020 8220 6000, email admin@ageukrbh.org.uk, or use the referral/contact form on our website. We also welcome volunteers who want to help older people access services, share skills, or provide companionship.

Service area: Please note, this service is only available to residents within the catchment area of Age UK Redbridge, Barking & Havering (Redbridge, Barking & Dagenham, and Havering).

Residents of Havering, Redbridge or Barking & Dagenham aged 50+.
