The New Forest PCN Social Prescribing service helps adults registered with Wistaria & Milford Surgeries, Chawton House Surgery, Lyndhurst Surgery, or New Forest Central Medical Group to improve their wellbeing through non-medical support. Social prescribers offer personalised plans, connecting individuals to local and online community resources and groups, including healthy lifestyle activities, arts, music, befriending, counselling, housing, benefits, financial advice, employment, training, volunteering, and education. The service aims to reduce loneliness and social isolation. Delivery includes in-person group sessions at Chawton House Surgery, virtual group sessions via Zoom, and community wellbeing walks. Patients can self-refer online via the Joy web-based platform or through their GP practice.
