Maximising Independence Service

The Maximising Independence Service is a therapy-led, multidisciplinary team supporting adults aged 18 and over in Sutton. Our team includes nurses, physiotherapists, occupational therapists, assistant practitioners, rehabilitation assistants, and healthcare assistants, working closely with your GP or acute frailty team.

We help people with physical or medical conditions that affect their ability to function day-to-day, such as walking, balance, or hand function. Our goal is to assess your needs and provide tailored interventions to promote independence and improve your quality of life.

How we help:

  • Comprehensive assessment and management plans focused on your personal goals
  • Support for frailty, falls prevention, and domiciliary physiotherapy
  • Collaboration with you and your healthcare providers to maximise your independence

Our vision: Working with you to live your best life.

If you or someone you care for is struggling with daily activities due to a physical or medical condition, our service can help you regain confidence and independence at home and in your community.
