The Care Coordination Centre provides a single point of access for telephone enquiries and referrals for the adult community services delivered by North Kent, serving Dartford, Gravesham, Swanley and Swale. The centre acts as your first contact to help you access the right service quickly and smoothly. You can phone 0300 247 0400 (8am–8pm, seven days a week) or email VCL.DGSS-CCC@nhs.net with your referral or enquiry. If the centre is closed, please call NHS 111.
