The Community Nursing Service and Night Nursing Service provides compassionate, professional care for adults who are unable to leave their homes in Dartford, Gravesham, Swanley, and Swale.
Our dedicated teams offer a complete range of clinical assessments, care planning, and regular reviews, working closely with patients and their families to ensure the best possible support. We understand the challenges of managing health issues while housebound, and our service is designed to make life easier and safer for you.
- Clinical interventions include: wound care, intravenous therapy and management, palliative and end-of-life care, and catheter management.
- Emotional and physical support is provided every step of the way, ensuring you feel cared for and listened to.
- Availability: Our day service runs from 8am to 8pm, and our night service from 8pm to 8am, seven days a week.
- Local teams: Each team is aligned with a local GP practice and led by a district nurse, supported by registered nurses and care support staff.
Referrals and enquiries are simple and stress-free through our Care Coordination Centre. We are here to help you or your loved ones remain safe and comfortable at home, with expert nursing care available around the clock.
