Our Community Cardiology and Heart Failure Service provides dedicated support and case management for adults living with heart conditions, including heart failure and left ventricular systolic dysfunction. We deliver care in the comfort of your own home and in other community settings, making it easier for you to access the help you need.
Our compassionate team of nurses, led by a clinical nurse specialist, works closely with therapists and care support staff to offer comprehensive cardiac rehabilitation, heart failure management, and cardiac diagnostics. We focus on your unique needs, ensuring you receive the best possible care and support every step of the way.
