Neurological Rehabilitation (North Kent)

This service provides comprehensive neurological rehabilitation for adults (18+) in the Dartford, Gravesham, Swanley, and Swale areas of Kent. It supports individuals who have experienced a new neurological event or an acute change in an existing neurological condition, including stroke, traumatic brain injury, multiple sclerosis, Parkinson's disease, and motor neurone disease. The service aims to restore function, enhance quality of life, and improve mobility, self-care abilities, communication, cognition, and independence. A multidisciplinary team offers nursing, physiotherapy, occupational therapy, speech and language therapy, and dietetics. Care is delivered through inpatient rehabilitation wards at Gravesham Community Hospital and Livingstone Community Hospital (Dartford) for those requiring intensive support. Community-based rehabilitation is also provided in patients' homes, day centres, or outpatient clinics. Referrals are accepted from health or social care professionals. A GP or Consultant referral may be required for individuals not previously known to the team. Patients must be registered with a GP in the local area, have a confirmed neurological diagnosis, and be motivated to participate in a goal-directed rehabilitation process.
