Connect Well provide Social Prescribing and Community Navigation services for vulnerable Adults across East Kent.
Clients can self refer over the phone by calling our hub team at the number provided on our flyer and website.
We would encourage professionals to query whether a referral is within our remit if it is particularly unique - our phones and email is monitored on standard business days Monday - Friday , 9-5.
We accept all referrals for people 55+ , assuming they are within the remit of the support we are able to provide (ie. Benefit Maximisation , Social Housing and general signposting)
We also accept referrals for people aged 18-55 if they have a complex need , complex needs could be mental health conditions , hearing/visual impairments , or multiple long term health conditions.
