Social Prescribing (Aylesbury Central PCN)

Aylesbury Central PCN's Social Prescribing service helps individuals in the Aylesbury area to improve their health and wellbeing by connecting them with local community support. Link Workers provide free, confidential one-to-one appointments to discuss issues such as social isolation, housing, benefits, finance and debt, caring roles, disability support, employment, and non-medical health and wellbeing concerns. They also offer local support information and assistance with neurological assessments. Referrals can be made by GPs, nurses, other healthcare professionals, or by self-referral. The team can also arrange telephone calls to discuss support needs.
