Health and wellbeing coaches support individuals over 18 years old who are registered with a GP in the Aylesbury Central Primary Care Network area to improve their health and overall wellbeing. This includes those with a BMI over 25 (or 23 for individuals from the BAME community), Type 2 diabetes, or prediabetes. Coaches help set goals, provide support, offer guidance, encourage self-care, and provide resources and referrals, using techniques like motivational interviewing and visualisation. They cover areas such as motivation, stress management, self-worth, eating, exercise, managing long-term conditions, social health, and sleep. Sessions are typically one-to-one, lasting up to 60 minutes, and can be delivered face-to-face at your GP practice or a local community space, via video consultation, or over the phone. Individuals can self-refer or be referred by any staff member within their GP practice.
