Connects individuals to practical and emotional community support through social prescribing link workers and community wellbeing coaches. The service addresses issues such as loneliness, isolation, stress, money worries, housing problems, and managing long-term conditions. Link workers are based in GP practices and accept referrals from local agencies. Community Wellbeing Coaches offer one-to-one support to help individuals develop healthier behaviours and lifestyles. Various classes and groups are also available at Stocksbridge Community Leisure Centre. The service covers Stocksbridge and the Upper Don region, including Stocksbridge, Oughtibridge, Deepcar, Wharncliffe Side, Bolsterstone, Midhopestones, Upper Midhope, and Ewden.
