This service is designed to make it easy for professionals to refer individuals who may benefit from:

- Home adaptations and equipment
Key features:
- Referrals are screened the same day (Monday to Friday, 9am–5pm) by the Adult Care Assessment and Triage Team (ACATT).
- Professionals can attach supporting documents (e.g., reports, photographs) to help with the referral process.
- Guidance is provided for cases involving safeguarding concerns, assistive technology needs, and emergency out-of-hours support.
- Emergency out-of-hours support is available countywide from 5pm to 9am on weekdays and 24/7 on weekends and bank holidays.

- Use the online referral form for routine professional referrals.
- For urgent or emergency situations, contact the out-of-hours team by phone.
- For safeguarding concerns, use the dedicated safeguarding referral form via the Derbyshire Safeguarding Adults Board.
