WorkWell Wyre Forest Network of Independent Practices PCN

WorkWell is a joint initiative from the Department for Work and Pensions (DWP) and the Department for Health and Social Care (DHSC), designed to help people with health conditions or disabilities stay in or return to work.

What does WorkWell offer?

  • Early-intervention work and health assessment.
  • Light touch, low-intensity support tailored to your needs.
  • Help to address work and health challenges and overcome barriers to employment.
  • Access to general support, information, guidance, and connections to local services via telephone.
  • Face-to-face work and health coaching available if required.

Who is WorkWell for?

  • Anyone aged 16 or over.
  • People living in or registered with a GP in Herefordshire or Worcestershire.
  • Individuals with the right to work in the UK.
  • Those experiencing barriers to employment due to health conditions or disabilities.

WorkWell supports people who are:

  • In work and at risk of being signed off sick.
  • Out of work for up to 6 months due to sickness and needing early intervention.
  • Out of work for longer and needing more complex support (signposted to other services).

How to access WorkWell:

  • Refer yourself or someone else by calling your GP surgery and requesting a referral.
  • Connect with your Primary Care Network (PCN) using the referral forms provided on the website.
  • If your PCN isn’t live yet, join the waiting list via the contact form.

Employers: If you have an employee struggling with health conditions at work, you can refer them to WorkWell with their consent. Download the practical guide for supportive conversations from the website.

Cost: WorkWell is a free service for eligible individuals.
