Community Pathways

Community Pathways offers a person-centred, outcome-focused, and time-limited assessment and enablement programme for people aged 16-64 with disabilities living in Wolverhampton. Our goal is to help individuals maximise their independence and live as independently as possible in the community.

Who is this for? The service is designed for people who:

  • Are aged 16 or over, have a disability, and have assessed social care support needs (including prevention).
  • Have a current support plan (or EHCP) in place at the time of referral.
  • Would benefit from intensive support to learn skills for independent living, reducing long-term support needs, and accessing local services.
  • Are currently using residential care or colleges and have been identified as needing enablement training to move into community-based services.
  • Have been assessed by a social worker and have the potential to achieve a tenancy or reduced support via enablement training.
  • Are in a crisis situation and may benefit from short-term enablement support.

How does it work? The service is delivered in the community and from your home, with home visits as needed. We:

  • Refer people to organisations such as ‘Enable’ Supported Employment Service, Telecare, and other community organisations.
  • Carry out initial fact-finding meetings.
  • Undertake travel training when appropriate.
  • Prepare time-limited programmes to signpost people into universal services.
  • Support healthy lifestyles and accommodation choices.

Availability: We are open daily from 7.30am to 10pm, 7 days a week. There is no charge to use the service, but support is time-limited based on your identified outcomes and goals.

How to access: Referrals must be made via Social Work Teams. Other referrals (Connexions, College, Self-Referral, etc.) must also go through Social Work Teams. The service is not designed for those moving directly into residential or nursing care.

Service access criteria:

The service is available to people who meet at least two of the following criteria:-

  • Aged 16 or over, have a disability and have assessed social care support needs (including prevention)
  • Has a current support plan (or EHCP) in place at the time of referral
  • Would benefit from intensive support to learn skills to enable them to:
    • live more independently 
    • reduce their long-term support needs
    • access services in their local community

  • Currently using residential care/residential colleges who have been identified through the review process to require a period of enablement training to facilitate a move into community based services.
  • Have been assessed by a social worker and have the potential to achieve a tenancy or reduced support via Community Pathways Enablement Training.
  • In a crisis situation who may benefit from short-term enablement support.
