Mental Health Practitioners play an important role within a PCN to proactively identify and work with people who are experiencing symptoms relating to their mental health. This can include low mood, anxiety, depression, low motivation, poor sleep, stress and people who may need education on how to improve their mental health and give the opportunity to learn new skills to manage their mental health symptoms.
For Mental Health Practitioners patient empowerment is at the centre of interventions selected delivering a person centred approach to therapy and signposting to community resources to support integration into the community.
This is achieved by bringing together all the information about a person’s identified mental health needs and exploring therapeutic interventions and then reviewing there effectiveness. Also by signposting to relevant community resources and projects.
The Mental Health Practitioner role is embedded within the Primary Care Network, which aligns to the NHS Long Term Plan’s commitment to make mental health support accessible for all.
Mental Health Practitioners are unable to work with patients who are experiencing acute mental health symptoms such as psychosis or suicidal ideation with an immediate risk. We are unable to work with patients open to CMHT, secondary services such as the memory clinic or patients open to NHS Talking therapies. We are unable to work with patients where their primary need is substance or alcohol use.
Currently our wait for triages assessment in some surgeries is up to 14 days.
