CYP Co-ordinator Service

The CYP Co-ordinator works alongside children, young people, and their families to provide tailored support for mental health and wellbeing needs. The service offers holistic, non-clinical help, guiding patients to the right services, community resources, and early interventions. By collaborating with GPs, practice staff, and local partners, the Coordinator ensures joined-up care, smooth referrals, and timely support to promote positive health and wellbeing outcomes.

Once you have completed the Joy referral form please book a slot directly into the CYP Co-ordinators EMIS schedule, by following the steps below:

  • Under Find Slot, select 'GP Connect Slot'
  • Select from Organisation Group drop down 'SynergyPCN_GPConnectDirectBooking' or 'ColneUnionPCN_GPConnectDirectBooking' 
  • Change data range and select 'CYP Slot'

Your referral will only be complete and confirmed once you have completed the steps above.

Service access criteria:

Child or Young Person must be: 2-17 years old, registered with a South West Neighbourhood GP Practice, experiencing low to moderate emotional or mental health concerns, e.g. anxiety, low mood, isolation, or difficulties engaging at school, that do not meet the threshold for immediate Tier 3/4 or crisis services.
