Practice Plus Group MSK, Buckinghamshire offers clinical assessment and treatment for musculoskeletal (MSK) conditions. To access our services, you must be registered with a Buckinghamshire GP.
How to get referred:
- You can be referred by your GP, consultant, or another healthcare professional.
- For physiotherapy only, self-referral is available. Complete the self-referral form and send it to practiceplusgrp.music-referrals@nhs.net or post it to Unit 2, The Merlin Centre, Cressex Business Park, Lancaster Rd, High Wycombe, Bucks, HP12 3QL.
Once referred, we will contact you to arrange your first appointment at one of our eight convenient locations. You will receive a confirmation letter and relevant information about your appointment. A reminder text will be sent a few days before your appointment.
On the day:
- Please arrive on time. If you are late, let us know and we will do our best to accommodate you.
- Check in at reception, or if there is no reception, take a seat and your clinician will call you at your appointment time.
You will be assessed by a healthcare professional, who may provide same-day treatment and agree a management plan with you.
Referral pathways: All referrals are triaged according to evidence-based pathways for various MSK conditions (back, elbow, foot and ankle, knee, shoulder, wrist and hand). Please ensure recommended primary care interventions are completed and documented before referral.
