Exercise Referral Scheme

The Exercise Referral Scheme helps residents of Huntingdonshire become more active and manage health conditions through supported self-care. You will be guided throughout the scheme by a named Physical Activity & Exercise Specialist, who will work with you to develop and maintain a 12-week personalised activity programme. A large network of health professionals (GPs, physiotherapists, nurses, occupational therapists, etc.) from around 50 agencies can refer people to One Leisure facilities to access the programme. There are two entry points: a direct referral from a registered health professional, or a self-referral application. In both cases, eligible adults receive a tailored 12-week plan with regular check-ins and reviews with their Instructor to help you stay active and manage health conditions. What to expect: • A 12-week, personalised exercise programme with structured reviews • Access to guidance from a named Physical Activity & Exercise Specialist • A plan focused on rehabilitation and prevention of medical conditions, including conditions such as cancer, heart conditions, hypertension, diabetes, obesity, respiratory conditions, mental health, and musculoskeletal issues • Swims and gym access as part of options where appropriate (subject to timetable) Entry options and costs (as stated on the page): • Daytime option: Health professional referral £63 (one-off); Self-referral £77.50 (one-off); gym access Mon–Fri 6.30am–4.30pm plus weekend access and swimming (timetable dependent) • Anytime option: Health professional referral £85 (one-off); Self-referral £90 (one-off); unlimited gym access during opening hours, plus swimming (timetable dependent) • All-in option: Health professional referral £30 per month via direct debit (for 3 months); Self-referral £33.50 per month via direct debit (for 3 months); includes the above benefits plus access to all four One Leisure Centres, additional fitness classes (Self-Referral off-peak), and off-peak court hire Administration and access: • Health professional referrals are welcomed and there is no cost to referees • Health professionals refer using the Exercise Referral Form; self-referral uses the Self Referral Exercise Form • After completing the 12-week programme, participants can access the After Care Scheme for ongoing support (currently £10 for a 30-minute review and updated programme) Who is it for? • Exercise Referral (health professional): Any resident of Huntingdonshire over 16 years old referred by a health professional who will benefit from exercise for rehabilitation and prevention of medical conditions • Self-Referral: Any resident of Huntingdonshire aged 18 or older who will benefit from exercise for rehabilitation and prevention of medical conditions Delivery and venue: • Delivered through One Leisure centres with a named Physical Activity & Exercise Specialist guiding you through the programme • Includes fitness activities and swimming where appropriate This page also references related scheme documents (e.g., Exercise Referral Scheme Outline and Self Referral Scheme Outline) and an After Care Scheme for participants who have completed the initial programme.
