We provide personalised floating support across Lewisham for people with mental health needs, including those living with dementia, psychosis, neurodiversity, or substance use challenges.

Our friendly team can visit you at home, in supported housing, or in the community, helping you build confidence, stay well, and connect with local opportunities.

How We Support You
Alongside wellbeing monitoring, medication reminders, and tenancy support, we specialise in social prescribing activities such as:

  • Joining community groups and clubs
  • Accessing exercise classes or walking groups
  • Volunteering and employability opportunities
  • Creative activities like art or music sessions
  • Support to engage in education or training
  • We can also help with hospital discharge, attending appointments, shopping, and everyday tasks — making sure you have the right support, at the right time

People over the age of 18 who don't require personal care (regulated activities) as part of our support.
