Forest Heath PCN is a collaborative network of local GP practices working together to provide integrated, proactive, and personalised health and social care for people in Newmarket, Mildenhall, Brandon, and surrounding villages.
We serve a combined population of 71,000 patients through our member practices: Brandon Medical Practice, Forest Surgery, The Surgery – Lakenheath, The Market Cross Surgery, Oakfield Surgery, Orchard House Surgery, The Reynard Surgery, and Rookery Medical Centre.
Our services include:
- Access to a Cancer Care Nurse for patients (contact your surgery for details)
- Social Prescribing and Health & Wellbeing Coach referral services
- Support for long-term conditions such as diabetes, heart disease, and mental health problems
- Collaboration with community, mental health, social care, pharmacy, hospital, and voluntary services
Our approach: We focus on proactive care, working closely with patients to understand what matters to them and to improve their health and wellbeing. By working together, we can employ more specialist clinical staff and offer a wider range of services than individual practices alone.
