Neighbourhood Navigation is a social prescribing service from First Community Health & Care, designed to help adults in East Surrey connect with local, non-medical community-based services to improve overall wellbeing.
Many daily challenges, such as stress, financial difficulties, social isolation, and loneliness, can negatively impact your health. Medication isn't always the answer. Our neighbourhood navigators work with you to understand your needs and help you access the right local support, whether that's joining a group, club, or activity, or getting help with practical issues.
- Who can use the service? Anyone aged 18 or over living in East Surrey can be referred by their GP or another healthcare professional.
- How does it work? After referral from your GP or other health care professional, a neighbourhood navigator will contact you, spend time understanding your needs, and help you take the first step towards accessing local support. They can signpost or refer you directly to services that can help.
- Self-help option: You don't always need a referral to our team, you can explore the Joy online marketplace to search for local groups, clubs, and activities that suits your interests and needs.
- Our team: Five neighbourhood navigators and a communications & engagement officer work closely with local GPs and health professionals to support you.
