The Winter Wellness Scheme is a community-focused initiative by Havering Council, in partnership with the NHS and local organizations, to support residents in maintaining good health and wellbeing during the winter months.
This program offers tailored wellness calls to residents, helping connect them to health, care, and community services designed to meet their individual needs.
Our goal is to ensure that everyone has access to the support they need to stay healthy and independent.
The scheme connects eligible residents with resources and services aimed at supporting physical and mental health, fostering independence, and addressing unique needs.
By working closely with the NHS, local area coordinators, social prescribers, and community organisations, we ensure a collaborative approach to wellness.
You can use this form to suggest residents that the team should be getting in touch with. Our team get in touch with residents in order of risk. If a resident doesn't meet the risk threshold, we will let you know.
The residents that are the highest risk would answer 'yes' to all of the below:
Financial Risk :
1. If you are a parent of a child, have you applied for Free School Meals ?
2. Are you in CTAX arrears ?
3. Do you live in a council property ?
4. Are you on Single Person Discount ?
Personal Risk :
1. Do you have any Disabilities ?
2. Are you retired ?
3. Are you over 80 years old ?
4. Do you have an Open Social Care case ?
5. Has there been a bereavement in your family in the last 3 month ?
6. Have you recently moved in the borough ?
7. Are you unemployed ?
8. Are there less than 3 adults living at the property ?
9. Do you have assisted waste collection ?
10. Do you have a freedom pass ?
