PCN Dietitians
Ø There are three dieticians - Jenna Colin, Malgorzata Szychta and Marianne Williams.
Ø They offer a combination of face to face and telephone clinics. Currently face to face clinics are based at: Nuneaton - Old Mill Surgery , Red Roofs Surgery , Arbury Medical Centre , Manor Court Surgery, Stockingford Surgery and Bulkington - Rugby Road Surgery
Ø They are looking for further clinic rooms particularly on Mondays or Wednesdays and in the Bedworth or Bulkington area.
Ø No home visits.
Ø They mainly cover patients with Diabetes, Weight Management, Cardiovascular Disease, Gastroenterology, Malnutrition and Food intolerances.
Ø Any member of the PCN can refer. The referral form is located on the EMIS Referral Tree and is named “PCN Dietitian Referral Form “.
Ø Alternatively, email patients name, NHS number, practice and referral reason to wicb.dietitian@nhs.net .
Ø Voicemail for patients: 01926567178 - Patient can also email us on the same email address
Ø No self referrals from patients.
