CRT Together - Improving The Cancer Journey

CRT Together - Improving the Cancer Journey programme is a service delivered by the Trust in partnership with Macmillan Cancer Support. The aim of the programme is to deliver a conditions specific non-clinical service across the Cwm Taff Morgannwg University Health Board region. Information, advice and support will be provided to participants diagnosed with cancer. CRT Together will support people, who have a diagnosis of cancer, to complete an Electronic Holistic Needs Assessment (e-HNA) and offer holistic and practical support to address the needs identified. We will also support carers’ and family members to offer help and guidance to a loved one who may not be able to access services themselves (this will only be carried out when the person living with cancer gives permission for us to do so). Information supplied will only be used to deliver the service.

How your information will be used by CRT Together: This form will be used to start the sign-up process onto the CRT Together service and arrange your first appointment. Information supplied by you throughout the CRT Together service will only be used to deliver the programme and for the purposes of statistical analysis. Sensitive data will be collected to help determine appropriate support. 

 

Your details will be securely stored on: our servers, eHNA, and Joy (our client database). Where electronic signature is used, they will also be stored on Legalesign. All information is maintained confidentially and is not sold to third parties. Where activities are funded and/or delivered in partnership with external organisations, we may share the information you have given us with them, which may include personal information. For a list of funders/partnerships please contact gdpr@coalfields-regen.org.uk. Personal data is not disclosed to any third party without your consent. However the wellbeing and safety of yourself and others’ is paramount to the service and should you disclose information about yourself or anyone else which raises concern about the possibility of harm to anyone, we may be entitled to bring this information to the attention of all appropriate agencies.
