Thrive Social Prescribing for Young People

We are a simple solution to a complex need. 

Our Thr1ve Social Prescribing Wellbeing Mentors work 1:1 with young people (14-18) and young adults (19-25) to discover what social issues are affecting their health and wellbeing, centring on a holistic approach that identifies their individual needs.  

Working alongside young people, we offer short term support which is sustainable, to try new activities, develop independence skills or explore volunteering opportunities, supporting them to access their local communities and make wider social connections.

We supporting those young people who may exhibit signs of loneliness, social isolation, social anxiety, low-level mental health challenges, or those who want to engage with education, employment, or training. 

 All we ask is that the young people want to make positive changes to their lives.

Any young person can be referred into the service aged 14-18 or up to the age of 25 with additional needs as long as they are registered to a doctor’s surgery in one of the following Primary Care Networks:

    • North Hardwick and Bolsover PCN* 
    • North Derbyshire PCN*

 

Or live in the following areas:

    • Chesterfield Borough Council area
    • North East Derbyshire District Council area
    • Bolsover District Council area
    • Glossop
    • Buxton
    • North Amber Valley area
