Thr1ve Social Prescribing for Young People offers a free, short-term, person-centred support for young people to improve mental health and wellbeing and connect with their communities.

The service provides both 1:1 support and small group sessions (in certain areas) for:

• Young people aged 14–18

• Young adults aged 19–25 (no formal diagnosis needed; additional support needs accepted).

We offer an open referral pathway.  They can be self-initiated or from parents/carers, school staff, school nurses, GPs, or other professionals. A typical pathway involves an initial assessment and a personalised plan, with options for accompanying the young person to community groups or activities.

As a guide, we support young people for 8 sessions, although more or less as needed;

Areas covered include Chesterfield, Glossop, Buxton, North-East Derbyshire, Bolsover and Amber Valley and Derbyshire Dales around Bakewell, Matlock and Wirksworth.

We support all young people who are struggling with their emotional health and wellbeing, as long as they feel ready to make positive changes to their lives.  Although priorities include Care Experienced Young People, Young People Not in Education, Employment or Training, Young Carers and Young Adult Carers, and those with Refugee or Asylum Seeker status..

Who can be referred:

Young people registered with GP practices under North Hardwick and Bolsover PCN; North Derbyshire PCN.

Also supports young people living in Chesterfield, Glossop, Buxton, North-East Derbyshire, Bolsover, Amber Valley, Wirksworth, Matlock and Bakewell (and surrounding areas).
