Maximising Independence Service

We are a multidisciplinary team made up of a nurse, physiotherapists, occupational therapists, assistant practitioners, rehabilitation assistants and healthcare assistant working in conjunction with your GP or acute frailty team.

Our aim is to assess and provide intervention for those people who have a physical/medical condition which affects their functional ability, such as walking, balance, hand function etc.  

We collaborate with patients to establish a tailored management plan and work towards patient-centred goals to promote functional independence and improve quality of life.

We have three pathways which include Frailty, Falls Prevention and Domiciliary Physiotherapy.

This service is available for people registered with a Sutton GP. Referral can be made via Single Point of Referral via your GP or health and social care professional.

Registered with a Sutton GP

For patients under the age of 65, they would need to be housebound and a physiotherapy need only. For patients over the age of 65 the criteria is to have a fear of falling, or history of falls, decline in mobility and require occupational therapy, physiotherapy and/or nursing needs.
