What are social prescribers?

Social Prescribers work in GP practices and are part of the NHS. 

Social Prescribers, often referred to as community linkworkers, are here to:

  • listen to you, and put you in touch with the people and activities that might
  • help you to feel better
  • help you to connect with community activities, groups and services
  • creating a personalised care plan with you that supports your social and
  • emotional needs, that affect your health and wellbeing, such as housing and money 

The Social Prescriber will discuss with you ways to support you to achieve your health and wellbeing goals.

What do Social Prescribers do?

Social Prescribers are here to help you with non-medical related problems, including, but not limited to:

  • Loneliness
  • Bereavement
  • Housing and homelessness guidance and signposting
  • Financial issues and benefits guidance
  • Support for carers
  • Drugs and alcohol signposting
  • Help using computers or getting internet access
  • Domestic abuse
  • Support in Ageing Well
  • Access to food banks and local pantries
  • Low level mental health support (such as anxiety, panic attacks, low mood and
  • stress)
  • Smoking Cessation
  • Weight management guidance and signposting
  • Work related issues and volunteering

Social prescribing can help you to have more control over your own health and find ways to improve how you feel in a way that suits you.

Social Prescribers can refer you into local services that can help you with the problems you are facing.
