Care Coordination Centre

Care Coordination Centre provides a single point of access for telephone enquiries and referrals for North Kent Adult Community Services (excluding community wards). It directs enquiries and referrals to the appropriate clinical team to support timely and effective management, and provides a clear contact point for ongoing inquiries and redirection as needed. Operating hours are 8am to 8pm, seven days a week. General enquiries can be made by phone or email during office hours, and if you require urgent medical help outside these times you should contact NHS 111 (or 999 for emergencies).
