Community Respiratory and Home Oxygen Service

Community Respiratory and Home Oxygen Service provides specialist assessment and management for people living with chronic lung conditions. Led by a clinical nurse specialist, the team offers thorough assessments, including spirometry, oxygen assessments, and tailored treatment plans to help you breathe more comfortably and stay independent at home. Services are available at clinic venues or through home visits, with a patient-centred approach designed to be warm, supportive and easy to understand. Our services are available Monday to Friday, from 8am to 5pm. What we offer: • Urgent unwell service with same-day reviews for unwell patients (Monday to Friday 08:30–16:00) • Routine patient assessments for housebound patients (home visits) including clinical objective assessment, spirometry and pulse oximetry • Clinic venues at Sittingbourne Memorial Hospital and Sheppey Community Hospital offering: routine patient assessments in clinic; oxygen assessment clinics with capillary blood gas sampling; telephone advice How to access this service: • A referral is required from your GP or another healthcare professional • Once the referral is received, the Respiratory Team will triage it and you will be sent a letter inviting you to attend a clinic at one of the locations listed above, or a phone call to arrange a home visit as appropriate Additional information: • The Care Coordination Centre helps to make referrals or enquiries as simple as possible • A referral form is available on the North Kent Adult Community Services site and can be emailed to vcl.dgss-ccc@nhs.net • The service operates Monday to Friday, 8am–5pm Cost: Cost information is not stated on the website.
