What does this service provide?
Our team provides nursing and therapy, education and support to patients and their families and carers who have had a new neurological event or acute change in an existing neurological condition.
The service is available from Monday to Friday, 9am to 5pm, in Dartford, Gravesham, Swanley and Swale areas.
We work with you to identify rehabilitation goals for a set period of time, usually around 6-8 sessions, before reviewing their progress and discussing plans for the future.
We are here to help and support you every step of the way on your journey to recovery.

Preventing admission to hospital
Helping people to remain at home following a hospital admission
Giving advice to help people cope with a specific difficulty
Helping a person to make informed choices on the management of their condition
Helping increase someone’s confidence and independence in looking after themselves
Supporting carers and family members
Helping people to access community services such as local leisure centres, shopping, day centres, public transport
Supporting people in returning to work
Helping people to understand and manage their medication safely
Giving healthy lifestyle advice and nutritional support
Referral information
A health or social care professional can refer a person to the service. However if the person is not previously known to the team a GP or Consultant referral may be required.
People referred to the team must be registered with a GP in the local area, aged 18 years or over, have a confirmed neurological diagnosis and be ready and motivated to participate in a goal directed rehabilitation process.
If a person does not attend their booked appointment they may be discharged from the service.
