Community Neurological Rehabilitation Team

The Community Neurological Rehabilitation Team provides specialist nursing, therapy, education, and support for adults who have experienced a new neurological event or an acute change in an existing neurological condition.

Our friendly team works closely with patients, families, and carers to set personalised rehabilitation goals, typically over 6-8 sessions. We review progress together and help plan the next steps for ongoing recovery and independence.

  • Support is available Monday to Friday, 9am to 5pm in Dartford, Gravesham, Swanley, and Swale.
  • We help prevent unnecessary hospital admissions and support people to remain at home after discharge.
  • Our team offers practical advice for managing specific difficulties, making informed choices, and increasing confidence in self-care.
  • We support carers and family members, and help people access local community services such as leisure centres, shopping, day centres, and public transport.
  • We assist with returning to work, understanding and managing medication safely, and provide healthy lifestyle and nutritional advice.

How to access the service:

  • Referrals can be made by health or social care professionals. If you are not already known to the team, a GP or Consultant referral may be required.
  • To be eligible, you must be registered with a GP in the local area, aged 18 or over, have a confirmed neurological diagnosis, and be ready and motivated to participate in goal-directed rehabilitation.
  • If you do not attend your booked appointment, you may be discharged from the service.
