The Swale Rapid Response / Urgent Care Service provides fast, multidisciplinary support in the community to help prevent avoidable hospital admissions for adults experiencing a health crisis or carer breakdown.
What we offer:
- A two-hour response time for urgent situations, delivered by advanced nurse practitioners, registered nurses, physiotherapists, occupational therapists, and healthcare support workers.
- Support for patients recovering from ill health, including tailored rehabilitation programmes and equipment to promote independence.
- Care provided in the patient’s own home or in an intermediate care bed, aiming to keep people safe and comfortable in familiar surroundings.
- Home visits within two hours of discharge from hospital, supporting early and safe return home.
- Comprehensive falls assessment and rehabilitation to maximise mobility and independence after a fall or acute episode.
- Personal care packages for up to 3 days, crisis care at end of life, and urgent clinical interventions (e.g. catheter changes, IV antibiotics, wound management).
- Close partnership with social and primary care services to ensure seamless support.
Availability: Seven days a week, 8am to 8pm.
Who is it for? Adults living in Swale who are deteriorating, in crisis, or need urgent support to avoid hospital admission or to return home safely after a hospital stay.
