The Tissue Viability Service (TVN) provides expert support for adults with complex wound care needs across Dartford, Gravesham, Swanley, and Swale.
Our dedicated team, led by a clinical nurse specialist and supported by registered nurses, offers:
- Specialist assessment and management of complex wounds
- Education and prevention advice for patients and carers
- Care delivered in clinics, in the patient’s own home, and support for inpatient wards
Service hours: Monday to Friday, 9am – 5pm.
We work closely with other community services to ensure seamless, high-quality care. Referrals and enquiries are managed through our Care Coordination Centre, making access simple and stress-free.
Who can use this service? Adults registered with a GP in Dartford, Gravesham, Swanley, or Swale who require specialist wound care or advice.
Cost: This service is free for eligible patients.
